What to expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Please have your current insurance card and information, as well as a copy of all medications you are currently taking, and emergency contact information with you when you arrive.

Financial Policy: This form is provided to every patient of Greenbrier Valley ENT and Facial Plastic Surgery. Please read the financial policy and sign the last page (signature page). Bring the signed signature page with you to your first appointment. We will add this information to your chart:

Health Information: This information will be entered into your chart prior to examination by our providers. This is basic health information including medical and surgical history, family history, medications, and allergies, and symptoms of your current illness.

HIPAA: Please see our HIPAA Privacy Policy on the website. Print this policy out if you wish. Once you have read and understand our privacy policy print out and sign the New Patient HIPAA form. This form will provide our office with your personal information and will allow you to notify us of individuals with whom you wish to share your personal medical information. Your basic patient information (address, phone numbers, etc) will not be shared with anyone.

From Route 219 North, go past Greenbrier Valley Medical Center onto Stuart Davis Road. Continue around bend to the right and view Greyrock Professional Park on your left. Take the third left onto Dawkins drive (through two stone pillars) and we are the last brick building on the right.
