We know when it comes to scheduling an appointment convenience and speed are necessary. We have made requesting an appointment a simple process via our website. Patients are seen by appointment only, although walk-ins are accepted in the case of an emergency. Please do not use this website appointment request for emergency situations. If you are having an emergency please dial 911 or proceed to the nearest emergency room.
If you would like us to release your medical records to a third party you must complete a Medical Records Release Form.
If you would like us to disclose your health information to another individual or doctors office you must complete an Authorization for the Use and Disclosure of Protected Health Information.
